Please enter all names you wish to include in our 5786 Memorial Booklet.  Please separate names by either a comma or by hitting the return key before entering the next name. 

You may list as many names as you would like included.  
You may list individual names separated by either a comma or by hitting the return key between names.  

Example:   
Bob and Julie Smith
Jane and Fred Doe
Mildred Jones

Or you may list by family name. 

Example:  The Smith Family, The Doe Family, and Mildred Jones 
or 
The Bob Smith Family, The Fred Doe Family, and Mildred Jones. 

Please note that if many names are listed, formatting may be changed to best accommodate all names within the booklet.  All family names will be listed together as provided here.
